ACES

Written parent permission and written physician orders are required before any medication is dispensed during the ACES program (prescription or over the counter). Medications must be in their original container with proper label. Site Leads do not have access to the school nurse's office. Parents are responsible for transferring medication to their ACES site.

All applicable health forms must be filled out annually or as needed when changes occur such as dosage, medication change or time of day medication is administered.
